Community Health Survey

Live Healthy Lane is conducting a survey to inform the next Community Health Improvement Plan. The survey is open through February 28, 2019.

About the Community Health Improvement Plan
In 2017, out of 36 counties in Oregon, Lane County ranks 13th for health outcomes (length of life and quality of life) and 12th for health factors (health behaviors, clinical care, social & economic factors, and physical environment). Based on the findings from the Community Health Needs Assessment, the Lane County Regional Community Health Improvement Plan (CHIP) outlines how we’ll work together to make improve behavioral, physical, and oral health, including overall well-being.
