If you seek new patient services, please complete this confidential online intake form.  If you require assistance in completing the form, please call the front office at 541.683.1641 to schedule a time with a Peer Support Specialist who will help you.  You can also drop into our physical locations to pick up a paper copy of the form.  If you are seeking harm reduction services, you can come by the Harm Reduction Treatment Center building during our open hours without an appointment.

Fees: Payment is determined on a sliding fee basis with proof of income. We accept Lane County’s Oregon Health Plan (OHP) (Trillium and Pacific Source) and Federal and State grant funding for qualified individuals. No one is turned away based on their inability to pay, and we never send anyone to collections.
