Navigation Empowerment Services Team (NEST)

White Bird Clinic helps people with phone access, EBT card replacement, direct express cards, ID and birth certificate assistance, rental and security deposit assistance, and navigating housing lists, OHP, and addiction service referrals.

Our office at 323 E 12th Ave is open for:

Walk-in case management: Monday through Friday, 9:30 a.m. – 12 noon

Case management appointments: Monday through Friday 1:00 p.m. – 4:30 p.m.
(Some limited walk-in services may be available during these afternoon hours)

Assistance is also available by phone: 541-342-1295 (9:30 a.m. to 3:00 p.m.).

A new schedule of service availability begins on July 1 2025:

Mail:
Walk-up mail pick-up services will be offered Tuesdays and Thursdays from 1:00 p.m. – 4:30 p.m.
Clients can call for mail checks 9:00 a.m. – 5:00 p.m. Monday through Friday.
Clients who are actively engaged in case management may pick-up their mail at their case management appointments or arrange pick up appointments via their case manager Monday-Friday 9:00 a.m. – 4:30 p.m. (closed for lunch 12-1 p.m.)

Food:
Sandwiches and water will be offered on Tuesdays and Thursdays from 1:00 p.m. – 4:30 p.m.

Bus Passes:
Bus passes will be distributed to active case-managed clients as a part of their service plan when appropriate.

Eyeglasses Vouchers:
Eyeglasses voucher sign-ups for new participants will be offered on Tuesdays and Thursdays from 1:00 p.m. – 4:30 p.m.

Information and Referrals:
Walk-up information and referral services will be offered on Tuesdays and Thursdays from 1:00 p.m. – 4:30 p.m.

Front Door Assessments Monday and Friday 9:30 a.m. – 3:30 p.m. (closed for lunch 12-1 p.m.)

At Front Door Assessments, we conduct a series of questions with households to understand their experiences with homelessness and identify any barriers they may face in securing housing. Our highly trained professionals carry out these assessments, ensuring each assessment is conducted with expertise and care. The process typically takes 45-60 minutes to complete. After the assessment, we place eligible households on the Centralized Waiting List, where their priority is determined by the score they receive—a system that ensures fairness and equity, not on a first-come, first-served basis. Housing program referrals participating in Coordinated Entry are made directly from this Centralized Waiting List.

NEST is designed to help adults experiencing homelessness navigate available resources and sign up to receive assistance. Services offered may include assistance in accessing primary medical care, case management and advocacy, outpatient drug and alcohol treatment, group therapy, drug detox, mental health counseling and evaluation, Oregon birth certificate requests, and transportation to appointments.

Health Plan Enrollment

Assistance with applications for the Oregon Health Plan and help with successfully using health insurance is available.
