Now Accepting Applications for SDoH / Health Links Specialist

Define and develop a Health Leads based program in our safety net medical clinic.  We will partner with academic institutions, community social service agencies and primary care providers to help redefine “health care” to include access to food, housing, and other basic resources as an essential part of patient care for our homeless and low-income patients.

Responsibilities:

Social Determinants of Health Specialist (50%)

  • Provide Insurance Assisting (OHP) services for patients at Medical.
  • Provide limited SHIBA-style Medicare assistance for patients with Medicare at Medical.
  • Assist patients with accessing social services to improve their ability to meet their medical treatment goals.
  • Assist providers in understanding the resources available to their patients to better formulate effective treatment plans for their patients.
  • In conjunction with White Bird’s Helpbook! project, develop and maintain a resource database of current social services resources in Lane County.

Healthlinks Specialist (50%)

  • Using the model developed by Health Leads’, work with the Medical Program Coordinator and  Medical Director to design and implement a program which uses volunteers to support supporting our patients to access available local social services to support their medical treatment goals.
  • Oversee, train, develop, and motivate a team of undergraduate student volunteers to provide direct service.
  • Collaborate on design and implementation of standards for quality short term case management.
  • Communicate innovations and challenges to the Medical Program Coordinator and other program staff.
  • Monitor and facilitate volunteers’ communication with health care providers.
  • Ensure volunteers’ compliance with the program’s performance and reporting standards.
  • Manage limited client caseload to ensure continuity of Health Leads services during breaks in the academic calendar and appropriate management of complex cases.
  • Participate as a full member of the clinic team, including attending program meetings and acting as a resource for Clinic staff on the Health Leads project.
  • Ensure and track high quality service delivery.  Report data outcomes to the program on a monthly basis.

Requirements:

  1.  2-3 years of relevant experience required.
  2. Ability to work evenings and weekends as required to serve a student population
  3. Passion for White Bird’s mission and serving homeless and low/no income patients
  4. Demonstrated experience successfully leading and managing a team of predominantly college age volunteers
  5. Excellent interpersonal and communication skills
  6. Strong organizational, time management and self-management skills, attention to detail, and a high degree of flexibility and self-direction
  7. A sense of humor!!

Preferred:

  1. Experience in working in a consensus environment
  2. Bi-lingual English and Spanish
  3. Proficient in Microsoft Office especially Word, Excel and PowerPoint
  4. Proficient or quickly able to learn NextGen EHR Data Entry and Reporting tools
  5. Knowledge or experience of the Patient Centered Primary Care Home model

Accountability:

This position reports to the Medical Program Coordinator

NOTE: All positions at White Bird Clinic have a minimum 6-month probationary period.

Compensation:

The salary starts at $17/hr and goes up to $18/hr after the successful completion of a 6-month probation. Up to 40 hours per week.
