LCSW - Medical/Behavioral Health Integration
Pay and Benefits:
Base salary is equivalent to $22/hr.
Providing behavioral health care to patients in our Medical Clinic.
This position is responsible to operate in a consultative role within our primary care team; provide recommendations regarding behavioral interventions to the referring primary care providers; and conduct brief interventions with referred patients on behalf of the referring primary care provider (PCP).
- Maintains a visible presence to the PCPs during clinic operating hours.
- Is available for “curbside” consultation by being in the clinic or available by phone.
- Is available for same day and scheduled initial consultations with patients referred by PCPs and PMHNPs.
- Provides a range of services including screening for behavioral health conditions, assessment and intervention to help patients improve their medical conditions. Conducts risk assessments, as needed.
- Assists in the development of the clinic’s behavioral health program and its integration into primary care including but not limited behavioral health focused practice protocols, group and individual visits, and classes.
- Maintains an up‑to‑date library of patient education materials for commonly seen problems. Identifies, reviews, and modifies educational materials for literacy level and cultural appropriateness.
- As a member of the Clinic’s crisis response team, assists Primary Care Providers with patients in acute crisis when requested.
- Facilitates and tracks referrals to specialty MH/SUD services, and when appropriate, support a smooth transition from specialty MH/SUD services back to primary care.
- Prepares brief consultant notes for the medical chart that explain assessment findings, interventions delivered, and recommendations made to the PCP.
- Maintains clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations.
- Aids in capturing program evaluation and fidelity measures.
- Attends clinic meetings as requested by Medical Director and Medical Program Coordinator.
- Maintains Licensure for Oregon LCSW.
- Ability to qualify for Oregon Qualified Mental Health Professional (QMHP) credentialing requirements as required by Oregon Administrative Rules (OARs) 309-019-0125.
- Ability to work with a wide range of patients including low/no income, and homeless adults.
- Willingness to participate in continuing education.
- Ability to work effectively with minimal supervision.
- Strong computer skills including Microsoft Word, Excel, & Email.
- Willing and able to document in our NextGen EHR.
- Complete annual blood borne pathogen training, yearly TB screening, and HEP B (vaccine series or titers).

- Bilingual in English and Spanish
- CADC or other experience in substance abuse treatment methodologies.
- Experience working with clients with dual diagnosis
- Experience in working in a consensus environment
- Experience working with Electronic Health Records, especially NextGen EHR.
This position reports to the Medical Operations Manager.
NOTE: All positions at White Bird Clinic have a minimum 6-month probationary period.
