The Panel Manager acts as the administrative point person at the clinic in providing an organized, population-based, data driven and multidisciplinary team approach, in which our primary care team jointly plans and manages the care of patients with chronic diseases using a disease registry to identify clients’ unmet care needs to gather summary information for care interventions, and to communicate with clients.

This position is being hired for 30-40 hours per week

Pay and Benefits:

$14.00/hr. plus sick and vacation as well as medical and dental insurance.

The Panel Manager acts as the administrative point person at the clinic in providing an organized, population-based, data driven and multidisciplinary team approach, in which our primary care team jointly plans and manages the care of patients with chronic diseases using a disease registry to identify clients’ unmet care needs to gather summary information for care interventions, and to communicate with clients.

Job Description:
Responsibilities:
1. Manage overall and day to day panel measurement, actively manage compliance, compliance monitoring, ongoing composition and weighting
2. Work with Providers and Back Office staff to correctly document Chronic Disease data in the chronic disease registry section of our EHR system
3. Coordinate patient satisfaction survey projects and facilitate data auditing as necessary
4. Provide ongoing panel support services, including running panel statistics queries
5. Generate, analyze and publish detailed panel statistics for internal & stakeholder usage
6. Track referrals and communicate regularly with Back Office Staff to improve the referral process and patient compliance.
7. Work with the Program Coordinator, Medical Director and Program Staff to continuously improve panel services and develop patient self-management options at the clinic.
8. Conduct “messenger activities” regarding appointments, health maintenance, labs, etc. This includes active reminders, follow-ups, and encouragement to get existing clients to participate in care.

Requirements:
1. Proven experience of setting up and/or maintaining a Primary Care Clinic patient panel, with a solid understanding of the Patient Centered Medical Home model.
2. Proven Experience or ability to learn NextGen sEMRs reporting
3. Knowledge of and/or experience working with Chronic Disease management and associated panel measurement criteria
4. Strong project management, attention to detail, written and oral communication skills
5. Proven leadership and relationship management skills
6. Ability to work independently and to a deadline
7. Strong Communication Skills
8. A sense of humor
9. Complete new employee orientation

Preferred:
1. Experience in working in a consensus environment
2. Bi-lingual English and Spanish
3. Proficient in Microsoft Office especially Word and Excel

Accountability:
This position reports to the Medical Office Manager.
NOTE: All positions at White Bird Clinic have a minimum 3 month probationary period.
