This position operates in a consultative role within a primary care team utilizing the PCBH Model; provides recommendations regarding behavioral interventions to the referring clinic’s primary care providers; and conducts brief interventions with referred patients on behalf of the referring primary care provider (PCP).

Job Description:

Responsibilities include:

  1. Introduce patients to role of Behavioral Health Consultant in the integrated care setting using the biopsychosocial model.
  2. Utilize appropriate assessment tools including PHQ-9, SBIRT and others and uses results to guide treatment plan.
  3. Identify referral problem and provide evidence-based interventions appropriate for the primary care setting.
  4. Coordinate care with primary care team members, including physicians, nurses, medical assistants, Behavioral Health Assistant and Health Links community resource program.
  5. Assist in the development of clinical pathway programs, group medical appointments, classes, and behavior focused practice protocols.
  6. Provide group interventions for patients in primary care for issues such as chronic pain management, stress management, depression/anxiety, smoking cessation, weight loss and others utilizing CBT, DBT, ACT, Mindfulness-based approaches and other evidence based approaches.
  7. Serve as a consultant on behavioral health issues to members of the primary care team both informally and through developing and presenting educational lectures and written materials to the team members.
  8. Attends clinic meetings, including program meeting and other meetings as requested by Medical Director
  9. Assist with maintaining an up-to-date library of patient education materials for commonly seen problems and a library of training materials for project staff.
  10. Facilitates referrals to other White Bird Clinic programs when appropriate.


  1. Licensure (or license-eligible within six months) in Oregon as a psychologist or Masters level counselor (MSW, MFT, LPC).
  2. Interest in and demonstrated ability to work as part of an integrated team of healthcare professionals including medical providers, other behavioral health providers, care managers, and community partners, to provide coordinated care to clients with a wide range of psychological needs.
  3. Excellent communication skills as evidenced by the ability to establish collaborative relationships with clients and other professionals.
  4. Ability to effectively deliver psychotherapeutic interventions that are evidence-based and can be applied to a wide range of clients.
  5. Demonstrated commitment to providing culturally competent services, with an ongoing commitment to training and development in this area.
  6. Training and experience delivering psychotherapeutic services to clients with complex medical issues and a wide range of behavioral health concerns.
  7. Demonstrated ability to accurately document interventions, including charting on clinical services, completing written psychological reports, and other professional documentation.
  8. Complete activities required to maintain a license to practice in the State of Oregon in good standing.
  9. Complete new employee orientation
  10. Minimum six-month probationary period
